Consultation Checklist

Prior to any consultation taking place, please make sure you have provided us with the following documents:

  • A complete copy of the insured’s long term care insurance policy.
  • Any applicable denial letters.
  • Any applicable policy lapse notices.
  • And and all invoices for home care and/or assisted living.
  • Any assessments or care plans generated by your home care company and/or assisted living facility.
  • Payment of the consultation fee.
  • If you are acting as power of attorney on behalf of the insured, please provide a copy of the power of attorney documents.
