Consultation Checklist
Prior to any consultation taking place, please make sure you have provided us with the following documents:
- A complete copy of the insured’s long term care insurance policy.
- Any applicable denial letters.
- Any applicable policy lapse notices.
- And and all invoices for home care and/or assisted living.
- Any assessments or care plans generated by your home care company and/or assisted living facility.
- Payment of the consultation fee.
- If you are acting as power of attorney on behalf of the insured, please provide a copy of the power of attorney documents.
